Our office staff will ask a few brief questions to learn about your son or daughter’s needs and goals, and gather basic information before scheduling a tour.

During the call, we will schedule a 15–20 minute tour at either our Roswell or Marietta location.
 

You and your son/daughter are encouraged to attend together so you can meet our team, see the workshop, and get a feel for the environment.
 

A confirmation email will be sent once scheduled.

At the time of your tour, our office can verify most major insurance plans in-house.

This is typically completed at the end of the tour, before scheduling your initial evaluation.

 Following the tour and insurance verification, we will schedule an initial evaluation with Occupational Therapy (OT), Speech Therapy (ST), or both.

Intake paperwork will be provided at this time as well.

All forms are available for download. If preferred, we are happy to email or mail them directly to you.

Please return the completed paperwork before or on the day of your child’s initial evaluation. You may submit it in any of the following ways:
